Ira.. am so sorry to here the trip troubles but am glad you posted this as I have always had good response from AMEX.

Wondered what would happen when I fly home to my Dad's in WY use my Amex for a Compact or midsize SUV. I figure just on frequency of use odds I am going to have an accident not to mention ice/snow, deer/antelope and the occasional farm equipment/big truck. I asked that Amex send to me Description of Coverage below.
Note that it says AMEX does not cover Full size SUVs or rentals outside of the US..suggest check with them again to see if this has chgd..if not I woud thank them greatly for handling all the headache until they figured out the other guy was at fault..

As requested, we have enclosed a Description of Coverage which fully explains the benefits and restrictions of this program. We recommend that you read all of the information, however, highlights of the program are noted below:

Use your eligible American Express Card to reserve and pay for the entire rental
Decline the car rental company's Collision Damage Waiver and Theft Protection
There is no liability coverage provided with your American Express Card
American Express provides Excess coverage to all other insurance policies
There is no coverage provided for vehicles rented outside of the United States, it's territories and possessions
Coverage is provided for vehicles rented for 30 consecutive days or less
There is no coverage provided for Loss of Use or Loss of Revenue
The Card member must be the primary renter of the vehicle
All additional drivers must be listed on the rental agreement
Cargo Vans, Custom Vans, Pick-up Trucks and moving vans such as U-hauls are not covered
Compact Sport Utility vehicles are covered, however, there is no coverage provided for full size sport utility vehicles such as a Ford Expedition
